Chronic Care Management is a program for Medicare and Medicare Advantage patients with two or more chronic conditions that helps keep track of your health and well-being between visits to the doctor. The program offers coordination of care for the patient, assists patients in managing chronic conditions, improves the overall quality of care, and helps reduce health care costs by eliminating unnecessary ER visits and hospitalizations.
