Our Diabetes Clinic team provides patient-centered care for people with Type 1 or Type 2 diabetes, gestational diabetes, or prediabetes.
Services provided:
- Education
- Blood-glucose monitoring
- Nutrition and exercise counseling
- Medication and prevention advice
- Help for special patient needs
Our team includes a nurse practitioner and a certified diabetes educator, who meets with patients at Gifford Health Center at Berlin, Bethel Health Center, Chelsea Health Center, and Kingwood Health Center.
Diabetes prevention and management classes are regularly offered through our Community Health Team.
