
BERLIN, VT — Gifford Health Care is pleased to welcome Elizabeth “Liz” Balaconis, AGPCNP-BC, to its Berlin primary care clinic, expanding access to comprehensive, relationship-based care for adult patients in the region.
Balaconis is a Primary Care Nurse Practitioner with experience spanning primary care, internal medicine, and inpatient psychiatry. Most recently, she served as a primary care nurse practitioner with Stanford Health Care in California and previously practiced at Central Vermont Medical Center and Harvard Vanguard Medical Associates.
For Balaconis, primary care is about seeing the full picture — not just addressing individual symptoms, but understanding how a person’s health connects to their daily life.
“In primary care, you get to know all about someone and how everything ties together,” she said. “You can piece together what might be happening based on their health, family life, sleep, and what’s most important to them.”
After working in larger academic health systems, Balaconis was drawn to Gifford’s collaborative culture and the accessibility of colleagues across specialties — an environment that supports thoughtful, coordinated care for patients.
Originally from Boston, Balaconis moved to Vermont in 2020 and now lives in Middlesex with her family. She earned her Master of Science in Nursing in Adult-Gerontology Primary Care from MGH Institute of Health Professions and holds a Bachelor of Arts in Human Evolutionary Biology from Harvard University. She is board certified through the American Nurses Credentialing Center and licensed as an RN and APRN in Vermont.
At Gifford’s Berlin clinic, Balaconis provides preventive care, chronic disease management, and acute visits for adult patients.
