For patients taking ongoing medication, your health care provider will want to see you regularly to monitor your health. Our goal is to provide you with a prescription at the time of your visit that will last until we see you again. If you need a refill between visits, please contact your Primary Care office location directly.

Patients of specialty providers should call their provider’s office directly.

When requesting a refill, please have the following information ready:

• Your full name
• Your phone number
• Your date of birth
• Your health care provider’s name
• The name and strength of the medication
• The directions for how you take the medicine (i.e. once daily at bedtime)
• The pharmacy that you use

Your refill request will be completed within 48 hours. Seven days advance notice is required for mail order prescriptions and all medications requiring pre-authorization by your insurance company.

Prescription Pick-ups at Gifford Primary Care

Prescriptions filled by Gifford may be picked up in the Community Health Clinic (located in Primary Care clinic area)

Monday – Friday: 8:30 a.m. – 12 p.m. and 1:30 – 4 p.m.
