For patients taking ongoing medication, your health care provider will want to see you at regular intervals to monitor your health. Our goal is to provide you with a prescription at the time of your visit that will last until we see you again. For patients needing prescription refills between appointments, we offer a prescription refill line.
To fill your prescription, we will need the following information:
- Your full name
- Your phone number
- Your date of birth
- Your health care provider’s name
- The name and strength of the medication
- The directions for how you take the medicine (i.e. once daily at bedtime)
- The pharmacy that you use
Your refill request will be completed within 48 hours. Seven days advance notice is required for mail order prescriptions and all medications requiring pre-authorization by your insurance company.

Prescriptions filled by Gifford may be picked up in the Community Health Clinic (located in Primary Care clinic area)
Monday – Friday: 8:30 a.m. – 12 p.m. and 1:30 – 4 p.m.
