Medications
When you visit your health care provider’s office, stay in the hospital or even come to the Emergency Department, bring your medications with you in a Ziploc-style bag. The actual medications give health care providers the most accurate list of what you’re taking, reducing the chance of medication errors, such as duplications or drug interactions. Your provider’s office can also provide you with a Medication List wallet card on which you can keep track of your medications, allergies and medical conditions.
Helpful tips
Keep your medications in one location so they can be easily collected. If you don’t have time to collect your medications yourself, designate a family member to bring the medications to the hospital for you.
