Michelle Wade, MSN/Ed, NP–C, APRN, has joined Gifford’s team of Hospitalist providers. She most recently worked as the practice lead for the nursing home services of Community Health Centers of the Rutland Region, and in the Intermediate Cardiac Care Unit at DHMC. She brings years of holistic bedside experience in both acute and long-term care settings, and specializes in adult and geriatric care.
Gifford’s Hospitalist Medicine providers—specially-trained physicians, nurse practitioners, and physician assistants—work collaboratively as a team to care for hospitalized patients and help with the transition back to the care of primary care providers.
“I enjoy working as a team to provide the extra support needed to help patients transition from one level of care to another,” Wade said. “I believe it is especially important to make time to talk with patients and their families during this fast-paced and stressful time, so we can develop a plan of care that meets the patient’s goals.”
Wade grew up in Maryland but came to Carrabassett Valley Academy at Sugarloaf USA in Maine as a sophomore in high school to ski for the USSA Championships. After working in the ski industry for 10 years, she began to train as a nurse, something she first enjoyed as a candy striper when she was 12 years old.
Board-certified by the American Academy of Nurse Practitioners, she received an Associate Degree in Nursing from VTC, a BSN and MSN/Ed from the University of Phoenix, and Post Masters Graduate Certification – Adult – Gerontology Nurse Practitioner (NP-C) from Felician University.
When not working she enjoys skiing with her granddaughters, camping, and her two dogs.
