RANDOLPH, Vt.,— Preparing for the unknown can feel daunting but your family, friends and health care providers need to know what your wishes are should you not be able to voice them. That’s why Gifford is hosting a Health Care Decisions Open House on Thursday, April 14th from 1-4 p.m. at Gifford Medical Center’s conference room.
This all-ages community event will allow attendees to learn more about the importance of designating a health agent and completing Advanced Directives. Gifford staff will be on hand to answer your questions, help guide you through the process of filling out your personal health care decisions and will discuss advanced care planning and end-of-life services.
“Filling out your Advance Directive is a gift you’re giving your loved ones, because they will know what your wishes are,” Community Health Team Lead, Lisa Delegato said.
Even if you already have an Advanced Directive, this open house would be a great opportunity to revise and make any necessary changes. For more information about the event please contact: Katja Evans, Community Relations Coordinator, at (802) 728-2377
