Free lifestyle change program info session January 13, 2018
Gifford Health Care’s Community Health Team is partnering with Vermont Blueprint for Health to offer the CDC-approved PreventT2 diabetes prevention program, which can prevent or delay type 2 diabetes.
Prediabetes (when blood sugar levels are higher than normal) puts people at risk for developing serious health conditions, including Type 2 diabetes, stroke, and heart disease.
Trained PreventT2 facilitators can teach you about small lifestyle changes—setting physical exercise goals, healthy eating, managing stress, and behavior modification—that can help reverse prediabetes. Participants in this free year-long program meet weekly for sixteen weeks followed by twice a month and then once a month meetings for a total of 22 sessions.
