At Gifford, we know that good health is about more than just medical care – it’s about supporting the whole person. My name is Anthony Knox, and I’m proud to serve as the Blueprint Program Manager for the Randolph Health Service Area (HSA). In this role, I oversee two vital teams that help patients address a wide range of health-related challenges: the Blueprint for Health team and the Health Connections team.
Whether you or a loved one are facing medical issues, financial stress, housing concerns, or navigating insurance coverage, our teams are here to help. Here’s how we work to support wellness in our communities:
Blueprint for Health
Our multidisciplinary Blueprint for Health team focuses on both medical and non-medical needs that affect overall well-being.
- Care Coordinators / Community Health Workers
These professionals help patients navigate Social Determinants of Health (SDoH), including food insecurity, unstable housing, and lack of transportation. While their services are non-medical, they play a critical role in supporting patients’ day-to-day lives. - Nurse Care Managers
These nurses assist patients with chronic medical conditions, helping them manage their health at home and avoid unnecessary emergency visits. - Social Worker
Located in our OB/GYN and Randolph Pediatrics departments, our social worker supports expecting mothers, new parents, and children. They provide traditional social work services as well as short-term therapeutic support during times of need. - MAT Nurse (Medication-Assisted Treatment)
Supporting patients in recovery, our MAT nurse works closely with our addiction medicine provider to deliver coordinated, compassionate care throughout the recovery journey.
Health Connections
The Health Connections team is dedicated to helping patients access and afford the care they need by assisting with:
- Health Insurance Enrollment
For those eligible for Medicaid, enrollment is open year-round. For commercial or Medicare supplement plans, assistance is available during the annual Open Enrollment Period (typically in November), or after qualifying life events. - Financial Assistance
We help patients apply for financial aid to reduce or cover medical costs, easing financial stress and making care more accessible.
